Integrative Evaluation
Our evaluation aims to assess and diagnose with a holistic focus. The 12th House Wellness values the mind body connection. Our aim is to do a comprehensive rebalance of our patient’s condition with an approach that considers the emotional, functional, spiritual, and social aspects of life.

Follow Up Visits
These visits are an opportunity to monitor progress and adjust the treatment plan as needed. This process is inclusive of medicinal and therapeutic interventions being assessed and modified to allow for integrative care that continues to aim for optimal patient outcomes.

Therapy
Therapy can be useful independently or in conjunction with medication. This is a valuable tool that can be applied with multiple methodologies. The therapeutic alliance between provider and patient is an important component to identifying the best approach.

Lab Testing
A holistic and integrative approach to your care may include lab work. This can help to assess for physiological abnormalities and streamline your comprehensive care.
Forms
Forms will only be filled out for well-established patients. Any form requiring evaluation and more than 10 minutes of provider time will be charged at a rate of $150
Currently we accept some insurance and hope to accept more in the near future. The insurances that we are in network with at this time include Aetna, Cigna, Quest Behavioral Health, Carelon Behavioral Health, Horizon Blue Cross and Blue Shield (BCBS) of NJ, and BCBS of MA - in NJ.
Alternatively, those without insurance or choosing to pay out of pocket may provide payment via credit/debit card or cash. Credit/Debit card information will be requested upon booking and will be run 24 hours prior to appointment time.
Insurance
In compliance with the “No Surprises Act” starting on 1/1/2022 “Good Faith Estimates” are available upon request and will be provided to all patients withing 3 business days of official request.
